This copy lacks pages 1-2 of the Appendix, which may have constituted the title-page. An extremely rare item, with one copy in the UK (Oxford), and two in the US (Yale and Newberry). Neither the Newberry, nor the Yale copy has the Appendix. The Oxford (All Souls College) copy does not seem to have the title-page to the Appendix either, which leads me to question whether there was ever one. ESTC N61579. Good.
