Front cover name/address in ink + stains, tears. I'd call it perfect-bound, black on brown covers with a brown cloth-tape backstrip. Front cover titling and decorative elements created by hand. Interior on inexpensive paper, typewritten & mimeographed, printed one side only (Title, Introduction, Contents, [1], 2 through 38 + last page blank). Interior lower margins throughout show moisture tide-mark. Introduction states use is soley for men in the Armed Forces of the United States and that the notes have been used in ESMDT classes in Anti-Aircraft Mathematics given at the University of California for men in the Coast Artillery. The following year UC published a hardbound Levy book on the same subject some 100 pages in length according to other sources. Lengthy UC "In Memoriam" biography of the author appears on-line. Fair.
