The text is from The Book of Common Prayer reformed according to the Plan of the late Dr. Samuel Clarke (1774), as well as Phillip Holland's A Form of Prayer and a New Collection of Psalms, for the Use of a Congregation of Protestant Dissenters in Liverpool (1763).
