Between pages 18 & 19 is a form to fill out with a friend's name & address, to get another copy of this book sent to them. As this form is inserted between pages 18 & 19 at the hinge, the end of the form sticks out btween pages 2 & 3. Possibly there was originally another form between pages 2 & 3? 1950
